Are you Covered?
*Due to the variety of plans available through each insurance company, patients are encouraged to call the number on their insurance card to find out if Zepf Services and Providers are covered on their specific plan.
Zepf Center is contracted with the following insurances for our BEHAVIORAL HEALTH services:
- Aetna and Aetna My Care Ohio
- AmeriHealth Caritas Medicaid
- Anthem BC/BS Ohio and Michigan, Anthem Medicaid, and Anthem Medicare Advantage (not in network with plan for Mercy Employees with ID# prefix of MRQ, not in network for UofM plans)
- Buckeye Medicaid, My Care Ohio, Marketplace, Buckeye WellCare by Allwell, and Buckeye Medicare Advantage
- Bureau of Workers’ Compensation (Ohio BWC)
- Carelon BH (formerly Beacon Health Options), limited providers
- CareSource Medicaid, My Care Ohio, Marketplace and CareSource Medicare Advantage*
- Frontpath*
- Humana, Humana Medicaid, and Humana Medicare Advantage*
- McLaren Health Advantage PPO, Medicare Advantage

If the insurance plan is different than those listed above, please call the number of the back of your insurance card to find an in-network/covered provider.
*IMPORTANT NOTE: Some Zepf services and provider types are never covered by Medicare or commercial insurance. Clients are responsible for the payment of all services not covered by their plan.
Most medical visits, assessments and therapy services by the following provider types are generally billable to Medicare and commercial insurance:
- Medical Doctors, Nurse Practitioners, LISW, LICDC, LPCC, and Clinical Psychologists.
- Services with all other Zepf staff not listed above are generally not billable to insurance, even if Zepf Center is contracted with their plan.
Zepf Center is contracted with the following insurance plans for the PRIMARY CARE/PHYSICAL HEALTH services provided in our Health Center Program:
