What do I need to bring to my initial Appointment/Intake?
For Behavioral Health Services:
Applications for Lucas or Wood County board subsidies for behavioral health services require documentation of household size and income, proof of residency, and an application for Ohio Medicaid if not already enrolled.
Patients are requested to bring to their initial intake assessment appointment essential financial items such as a valid picture ID, proof of household size and income, Social Security number, Medicare or Medicaid number, and private insurance policy numbers as applicable.
Acceptable forms of documentation for household size, income, and residency include the first page of your most recent IRS tax return, or your last two pay stubs and utility bill. We can help you apply for Medicaid coverage.
For Primary Care Services:
Applications for the Health Center Program Sliding Fee Discount Program require documentation of household size and income.
