Zepf Center Benefit Information
1. Health Care Benefits
A. Paramount: 1-800-462-3589, (summary of benefits form, enrollment application)
- Healthcare benefits are available for full-time and part-time employees. Part-time employees are eligible for single coverage. Full-time employees are eligible for family coverage; non-bargaining unit employees help pay a portion of the monthly premium. Full-time employees whose spouses are eligible for group coverage elsewhere cannot be covered under Zepf's healthcare.
- Changes other than qualifying events can only be made during the open enrollment period in December.
B. MetLife: (enrollment form, benefits summary for dental)
- Dental benefits are available for full-time and part-time employees. Part-time employees are eligible for single coverage. Full-time employees are eligible for family coverage. Full-time employees who elect more extensive coverage through the dental plan than the healthcare plan pay the difference in premium cost.
- Changes other than qualifying events can only be made during the open enrollment period in December.
C. Vision: (benefits summary, membership enrollment form, premium election form, optivue elitevue flyer)
- Vision coverage is voluntary and the premium cost is deducted from employees who elect the coverage's paycheck pre-tax. There is no card associated with vision coverage. Employees who choose this coverage must tell their qualifying eye care provider that they have vsp coverage before their appointment.
- Changes other than qualifying events can only be made during the open enrollment period in January.
