Hospital Team
The Hospital Team is a service provided to clients who have been admitted for inpatient treatment at one of the following hospitals: Flower, St. Vincent's, St. Charles and Northcoast Behavioral Health Care. Service is limited to existing clients and those assigned to the agency for services. The purpose is to promote continuity of care, assist with discharge planning, and coordinate linkage and ensure access to community outpatient services.
