Case Management
Case Management services assist and support people with long-term mental health problems to develop long-range life improvement goals. The Case Managers assist in maintaining housing, obtaining financial assistance and medical and psychiatric treatment, and linking with other community services including education, work and social programs. Services may include, but are not limited to, supportive counseling, coordination, referral, group activities, assessment, monitoring and ensuring access to emergency services.
