COMMUNITY SUPPORT PROGRAM (CSP)

Through the CSP at the Zepf Center, a case manager works with each person to achieve their goals. CSP assess their clients' specific strengths and needs and coordinate necessary services with other team members.

First, Individualized outcome-oriented Service Plans (ISP) are developed with each person. The CSP then facilitates the coordination of services to help the individual achieve his or her goals. CSP's assist in the development of plans, provide linkage to community resources, advocate for clients and actively engage them in identifying their needs.

With a holistic approach to helping people, our staff become involved in all areas of each client's life - frequently working with family members. The overall goal of the community support program is to improve each client's quality of community life and level of independence.

CSP services are offered at both the Central Avenue and Hamilton locations.
